Mission Statement

Our Lockport office strives to deliver progressive personalized care of the highest quality for each of our patients. We have friendly, bilingual personnel to service each of our patients.

Your First Appointment

When you come to the office for your first visit, you are encouraged to arrive ten minutes before the time of your appointment in order to allow ample time to prepare and register. New patients must bring their insurance cards and referral (if required) and a drivers license or state I.D. If an insurance card is not presented at the time of the visit, you will be asked to pay for the cost of that visit.

All co-pays must be paid at the time of service per your insurance contract.

All medical records pertinent to the visit, including labs, diagnostic imaging reports and office notes should be brought with the patient to their visit or requested to be faxed to our office by the referring physician.

We will ask you for a urine specimen at most of your visits with us.

Test Results

Lab results, results from procedures, X-rays or tests your physician has ordered are generally returned to our office within a one-to-two week time frame, the doctor then reviews them. Therefore, when you wish to learn the outcome of your test, we request that you call the office one to two weeks after the test has been performed and give us both a daytime and an evening number where you can be reached and our medical assistant will return your call. We will try to do our best to call regarding normal results as soon as possible. Please be advised that some test results may require a follow-up visit to our office.

Prescription Refills

We encourage you to call your pharmacy and have the pharmacist fax us during normal business hours for prescription refills. Please call well before you are out of your medication so that we have time to properly process your request. If you have not seen your physician for some time, it may be necessary for you to come in for a visit before your physician can refill your prescription.

On-Call Coverage
